Waiver and Liability Release Statement  Chase Community Venturers

      By participating in any event, program, or activity organized or sponsored by Chase Community Ventures  acknowledge and agree to the following:

1. Assumption of Risk: I understand that participation in Chase Community Ventures Activities may involve inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks associated with participation.

2. Release of Liability: In consideration of being allowed to participate in Chase Community Ventures programs or events, I, on behalf of myself, my heirs, assigns, and personal representatives, hereby release, discharge, and hold harmless Chase Community Venturers, its officers, directors, employees, volunteers, sponsors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained while participating in any activity or while on premises where activities are conducted.

3. Medical Treatment: I authorize Chase Community Ventures to secure emergency medical treatment in the event of injury or illness. I release and hold harmless the organization from any liability arising from such treatment.

4. Photographic Release: I grant permission for Chase Community Ventures to take and use photographs, video, or other media of me during activities for promotional, educational, or other purposes without compensation or further notice.

5. Compliance with Rules: I agree to follow all safety guidelines, instructions, and rules established by Chase Community Ventures and its representatives.
